Managed Care Team
Our PACE CNY Team strives to preserve and support the family unit. With PACE, the focus is on you and your family, and providing the best care possible for your loved one.
Your loved one will have a team of professionals working with them and your family to ensure they are getting the coordinated care they need. We realize that every individual has different needs, and our care team will take the time to get to know your loved one to understand what they need, and provide a personalized level of care to meet those requirements.
The interdisciplinary team that manages, integrates, and provides care is the key to the vigorous management of care that is at the core of the PACE Model.
Team members work together to assess need, plan treatment, provide care, oversee contract services, monitor care and the participant’s changing situation, and adjust the care plan as needed on a continuous basis. Care can be provided in a variety of settings— in the home, at a day center, and in a nursing home or a hospital.
At least semi-annually, the team reassesses each participant. The team also meets at the beginning of each day to review changes in participants during the previous 24 hours, and to make any adjustments to the care plan that may be indicated. Our team’s care planning is continuous.
