DAY Center CARE

The Day Center Manager coordinates care in the Day Center. Each month, the Recreation Therapy team develops a calendar offering a diverse variety of programs and activities with everything from dancing, music, and exercise class, to cooking, games, sewing, knitting, even computer classes, bingo, cards, and more. They also plan outside trips to shopping centers, restaurants, and special events like the New York State Fair, Lights on the Lake, and local sports outings (SU Sports and Syracuse Mets).

These activities and outings are designed to help participants learn new activities and do what they enjoy to the best of their abilities, while keeping any activity limitations or barriers in mind. Our goal is to help our participants maintain their individual purpose and enjoy the best quality of life possible.

Personal care is also available, and the Day Center offers a walk-in shower as well as a whirlpool bathtub, for those who have special personal care needs. Home Health Aides are available to assist with these activities. And there’s even a hairdresser on site (for an additional fee).

Our Registered Dietitian coordinates nutrition care by recommending appropriate therapeutic diets and nutritional supplements. The dietitians monitor weight, consumption of meals, and adjust menus in the day center to meet participants’ individual needs. Nutrition education is provided on an individual basis. PACE pays close attention to the individual nutritional needs of the participants we serve.

The Social Workers help coordinate the psycho-social needs with the participants, their families, and the PACE team. They are instrumental in helping address participant or family concerns as they arise. And, social work and dietician services may extend beyond the Day Center into the participant’s home or skilled nursing facility as needed.

PACE CNY has a very active Participant Council and Participant Advisory Committee, which makes recommendations for new programs and offers feedback to the PACE CNY Program so we can continually improve the satisfaction of our participants, as well as the quality of the programs we offer. PACE CNY works to provide a variety of programs that promote socialization, and make every day meaningful for participants.

PACE CNY has partnered with the New York State Department of Health Bureau of Occupational Health and Injury Prevention (BOHIP) as a sustainability partner in their Evidence-Based Falls Prevention Programs Financed by Prevention and Public Health Funds (HHA-2018-ACL-AOA-FPSG-0255). PACE CNY supports BOHIP’s efforts to establish an integrated, sustainable, evidence-based prevention program network to reach older adults and adults with disabilities with Tai Chi for Arthritis and A Matter of Balance by Dr. Paul Lam’s Tai Chi for Health Institute. Five staff members have completed the Instructor/Leader course, and have already led 15 participants through the first 16 session course.

PACE CNY Family Appreciation BBQ 2019:

Without the collaboration of family, care partners, and friends, PACE CNY could not fulfill its mission to keep participants in their homes and age in place with dignity and grace. Thank you!

Walmart and Dunkin’ Donuts! Shopping outings are an opportunity for participants to get out into the community, experience spontaneity, autonomy, and socialize with one another.

BIG DIP season!

Creativity flows in art programming at the day center. Participants are able to express themselves on an individual, or group level.

Tim held an Art Show on May 21, 2019 at PACE 2.   He displayed his work of paintings, poems, drawings, and pencil sketches.  He has been drawing at his kitchen table since he was 6 years old.   He has no formal training.  The picture being displayed is of his driver, Homer. He  wrote a poem to go along with the painting called Homer in the Moonlight.  His goal was to seeing the joy in others viewing his artwork.

Physical activities, like Aquatic Exercise, are offered in a community setting at Champions Fitness in Cicero. This low impact activity eases pain and increases endurance.

The PACE day center offers a diverse selection of group programming including baking and cooking. Participants are able to share recipes from their past, or try something new.

Participants are able to enjoy music in many different formats. From outside entertainers to a choir group, music ignites memories and gets people moving!
