Home Health Aides

HHAs are certified paraprofessional workers who are prepared to assist the sick or disabled and maintain the physical and emotional comfort of an individual in their “home” setting.

RN

The Home Care RN will participate in the planning, organizing, implementing, and evaluating of Home Care services, as well as provide direct nursing care to the participants in their home.

Drivers

Under the supervision of the Transportation Coordinator, is responsible for the direct transportation of participants who may be psychically or mentally limited to and from destinations. Position requires constant interaction with participants, participants’ families, and the staff of the Interdisciplinary Team.

Physical Therapists

Under the general supervision of the Director of Rehab, the Physical Therapist evaluates for physical therapy services, establish a treatment plan, provide appropriate treatment; provides necessary consultation and teaching to participants/family/caregivers and interdisciplinary team.
