One of the premier benefits of PACE CNY is our on-site medical care. We’re dedicated to the health and well-being of your loved one, and are happy to be able to offer a wide range of medical care and services with easy access in a central location.


Our comprehensive list of medical care and services include:

The PACE CNY doctor, nurse practitioner, and nurses are available from 8:00 a.m. to 5:00 p.m. Monday through Friday in the Clinic. There are treatment rooms, a medication room, and a room dedicated to massage therapy in the Clinic. Our participants can have blood drawn, IV therapy, x-rays taken, and cardiograms completed at the Clinic. The medical team reviews all medications upon enrollment.

Our team approach focuses on prevention and catching problems early, and we are committed to caring for participants in a non-institutional setting. PACE has a full complement of specialty medical physicians in the PACE CNY network; specialties include cardiology, pulmonology, rheumatology and others.

MEET Our Medical Staff


DAVID HEISIG, MD
PACE CNY Medical Director

Dr. Heisig graduated from Albany Medical College of Union University and completed his Internal Medicine training at SUNY Upstate Medical Center in Syracuse.  He was an Academic Gastroenterologist on staff at Upstate, and also served as the Residency Program Director for the Department of Medicine there.  Dr. Heisig joined Associated Gastroenterologists of CNY and practiced there for 10 years, and then returned to teaching as a Professor of Medicine at SUNY Upstate.

 


TWINKLE PATEL, MD

The structure of PACE CNY treatment, which is the multi-disciplinary team approach to elderly care and the interaction between patients and family, was the determining factor in Dr. Patel’s joining PACE CNY.  A graduate of medical school in Gujurat, India, Dr. Patel served her residency at Sacred Heart hospital in Allentown, PA, where she won the best residency award for scholarly achievement, professionalism and high moral and ethical conduct.  She is a member of the American Board of Family Practice.

 


SUZANNE LAMANNA, MD

Dr. Lamanna received her Masters from St Bonaventure’s and her D.O. from University of New England College of Osteopathic Medicine.  She completed her Family Medicine Residency training at St. Joseph’s Hospital in Syracuse where she also served as chief resident.  Dr. Lamanna worked in the St. Joseph Primary Care offices for five years before starting and maintaining her solo practice in Family Medicine in North Syracuse for two decades.

 


SALLY KLEMENS, MD

Sally Klemens, MD received her medical degree at the University of Pittsburgh School of Medicine. She completed her Internal Medicine residency at Upstate Medical Center and subsequently completed her Infectious Disease fellowship at the same institution. She then worked at Upstate Medical Center and VAMC with an academic appointment for 7 years. She has most recently practiced at St. Joseph’s Hospital for 24 years. Dr. Klemens joined the PACE program in July of 2022.

 


MARGARET WADE, PhD, DNP, FNP-C

Margie Wade joined PACE CNY staff in 2003 and states, “PACE is a program I would choose to care for a family member. Participants receive optimal care by a team of providers dedicated to each person’s well-being.”  Margie has more than 25 years of experience as a nurse practitioner. She received her doctorate of nursing practice degree from Chatham University and was a Clinical Assistant Professor at Upstate Medical University and Le Moyne College. She is board certified by the American Academy of Nurse Practitioners and was a past-president of the local chapter of the NYS Nurse Practitioner Association.

 

DESTA ANTHONY, NP

Desta Anthony is a graduate of Upstate Medical University and was the owner of the independent Nurse Practitioner house call practice, MobileMed.  In addition, she was a preceptor for the Nurse Practitioner program at LeMoyne College.  Desta is a member of the American Academy of House Call Providers and is board certified with the American Academy of Nurse Practitioners.

 

 


JENNIFER WINIARCZYK-NALLE, PMHNP-BC

Jennifer Winiarczyk-Nalle is a Board Certified Psychiatric Mental Health Nurse Practitioner.  A native of Syracuse, she has worked locally as a Licensed Practical Nurse, Registered Nurse and now Nurse Practitioner.  Jennifer has over 20 years of experience working with the geriatric population; 7 of those years dedicated to patients suffering from Alzheimer’s and/ or dementia.  Jennifer obtained her Master’s Degree from Stony Brook University.

 


CHRISTOPHER NORMAN, NP

Chris Norman is Board Certified Geriatric Nurse Practitioner and Holistic Nurse.  A native of the Southern Tier, he earned his Master’s Degree with Honors at Yale University, completed and Advance Practice Nurse Fellowship in the Geriatrics at NYU, and worked with University Geriatricians at Upstate Medical prior to coming to PACE.  Chris is Board Certified through the American Nurse Credentialing Center.

 

JESSICA LOKAY, NP

Jessica Lokay earned her Master’s degree with Upstate Medical University. She is a Board Certified Family Nurse Practitioner through the American Association of Nurse Practitioners.  A native of Syracuse, she has worked locally as a Registered Nurse in wound care, neurology and urology. She has volunteered for the Health Fair through the Syracuse Housing Authority, offering blood pressure screening, blood sugar checks and education about nutrition. She joined the PACE CNY staff in 2021 with goals of bringing the compassion of nursing to preventative and palliative medicine.

ANN MACWAN, NP

Graduated with Master’s in Nursing with a focus on Adult-Gerontology population from SUNY Stony Brook University. Completed her academic clinical practice requirements at SUNY Upstate in Syracuse, NY.  She holds a national certification with the American Academy of Nurse Practitioners.

BARBARA KANE, NP

Barbara Kane is a Board Certified Family Nurse Practitioner through the American Association of Nurse Practitioners.  A graduate of Upstate Medical University, Barbara brings over 35 years of medical experience to PACE.  Barbara has worked for Associated Gastroenterology of CNY and Syracuse VA Medical Center. In addition, Barbara has served as an Adjunct professor at Upstate Medical‘s College of Nursing and has lectured extensively for Pesi Healthcare.


COMMUNITY PHYSICIAN WAIVERS

Part of the PACE Federal Regulation requires that primary care be delivered by a PACE Physician or nurse practitioner.  PACE CNY was authorized to add up to ten physicians from the community using our approved community physician waiver.
