A Team Approach To Elder Care

PACE CNY has been providing services since 1997. It is one of only 1 of 151 PACE programs in 32 states and the District of Columbia.

PACE CNY provides a convincing blueprint for the future of eldercare, where an interdisciplinary team approach to senior care can actually improve quality of life in a cost effective manner.

The potential for improving quality of life is remarkable when you consider national PACE statistics, which mirror PACE CNY’s experience locally. A full 96% of older adults in the PACE program are 65 and older (average age of 83). They have an average of 8 diagnosed medical conditions including ailments like hypertension, arthritis, diabetes, dementia (45 percent, with another 30 percent showing cognitive impairment) and coronary artery disease.

At the heart of the PACE model is a team of geriatric healthcare specialists that provide in-depth evaluations and individualized care plans for each participant. Daily meetings allow the team to closely observe and manage all health and long-term care services for each individual. While nursing homes will always have a place in the total mix of services, families are often unaware of other options. Now, PACE CNY is your most flexible option!

The Center for Medicare and Medicaid Services (CMS) and the New York State Department of Health (DOH) conduct regular surveys to ensure a high quality of care.

With PACE CNY, your loved one can age in place in the comfort of the home they love.

PACE CNY is a unique alternative to nursing home care, providing a wide spectrum of care and services to individuals in various environments, meeting all their medical and social needs while enabling them to remain at home as long as possible. A comprehensive team —  primary care physicians, nurse practictioners, social workers, day care, rehab team, home care and dieticians are available to provide home care services in the participant’s home, day health centers, hospitals, or even long-term facilities if needed. On-call health care coverage by doctors and nurses is available 24 hours a day, 7 days a week, 365 days a year.

Think of us as one-stop shopping for all the health care needs you or a loved one may have. PACE CNY provides relief from the stress of caregiving with a program that provides and coordinates all types of care.
