‘Program for All-Inclusive Care for the Elderly’ in Central New York. It is a Medicaid and Medicare program for people age 55 and older who require nursing home level care and services but want to remain living independently at home.

PACE CNY Provides all the care and services covered by Medicare and Medicaid, as authorized by the interdisciplinary team, as well as additional medically-necessary care and services not covered by Medicare and Medicaid.

PACE CNY provides coverage for prescription drugs, doctor care, transportation, home care, checkups, hospital visits, and even nursing home stays whenever necessary.

You are eligible for PACE CNY services if you:

  • Are 55 years old or older
  • Live in Onondaga County
  • Are certified by New York State as needing nursing home level care
  • Can live safely in the community at your time of enrollment.

PACE CNY is an alternative to nursing care that allows older individuals to continue living in their homes and communities for as long as possible. It offers an integrated, comprehensive, coordinated health care program intended to keep participants as healthy and independent as possible for as long as possible.

Call PACE CNY at 315-452-5800 and ask to speak to an Intake Specialist who can thoroughly explain the PACE CNY program. If you are interested, we will schedule a “no charge” home visit to assess your needs and collect information to begin the assessment process.

Yes, if a person does not qualify for Medicaid, he or she is responsible for the portion of the monthly premium Medicaid would pay. PACE CNY staff will provide information on Medicaid eligibility requirements.

Yes. Once you enroll in the PACE CNY program, you are enrolled in a Medicare Part D prescription drug plan. PACE CNY pays for all prescriptions and non-prescription drugs deemed necessary by the interdisciplinary care team.

No. Day Center attendance is based on the needs and preferences of an individual.
