The PACE CNY Model

PACE CNY provides participants medical, rehabilitative, social, and personal services covered by Medicaid and Medicare. Services are provided by an interdisciplinary team of doctors, rehabilitation and recreation therapists, social workers, and dietitians who create and manage personalized care plans for each PACE CNY participant based on their abilities. PACE CNY services offer an alternative to nursing home care, allowing individuals to “age in place” by providing the care they need while they live at home.

Day Care Services

The Day Center features spaces for participants to receive personal and therapeutic care, participate in recreational activities, share meals, and socialize with friends.

In-Patient Care

PACE CNY will help coordinate hospital care when necessary. St. Joseph’s Hospital Health Center and Crouse Hospital are PACE CNY’s network providers.

In-Home Care

PACE CNY provides equipment approved by the Interdisciplinary Team to meet the care needs of PACE CNY participants.

Medical Care

PACE CNY offers on-site medical care at the health clinics located in both PACE CNY Day Centers in East and North Syracuse.

Medical Equipment

PACE CNY provides equipment approved by the Interdisciplinary Team to meet the care needs of PACE CNY participants.

Medication Management

PACE CNY provides all medications that the PACE CNY primary care or specialty physician providers prescribe, including over-the-counter drugs.

Nursing Home Services

If short- or long-term nursing home care is required, PACE CNY will help a participant transition from living at home independently to finding the appropriate nursing home care while remaining a PACE CNY participant.

Recreational Therapy

Recreational therapy is designed to help participants maintain their purpose, enrich their quality of life, and provide opportunities for friendship and connection.

Rehabilitation

The PACE CNY Rehabilitation Team includes physical, occupational, and speech therapists who work to improve the quality of life for all PACE CNY participants.

Transportation Services

PACE CNY provides free transportation to and from the Day Center, clinic, hospitals, specialty doctor appointments, and recreational therapy activities.
