PACE CNY has two locations – One location is in East Syracuse, and one in North Syracuse, New York.
Each facility features a medical clinic, a rehabilitation room for physical and occupational therapy services, and a Day Center where daily meals and social events occur. PACE CNY Participants are assigned to a location based on their address.
