How to Enroll

Step 1: Call PACE CNY

The state and federal governments have set eligibility and enrollment criteria. The intake nurse reviews this information with the potential participant to determine eligibility for the PACE program.

If the person is ineligible or not interested in the PACE model, information on other local programs that may meet an individual’s needs will be provided.

Step 2: Assessment

An assessment is required to qualify for PACE CNY services. PACE CNY can administer the assessment through Direct Eligibility, or you can obtain one through a New York Independent Assessor.

After your assessment, you can schedule a day to visit the PACE CNY Center that would be assigned to you based on your zip code. This day would include meeting the care team, rehabilitation and recreation therapists, dietitian, and a representative from the transportation department to help determine your needs.

Step 3: Determination of Medical and Financial Eligibility

A PACE CNY intake specialist will help you determine whether you or your loved one is medically and financially eligible for PACE CNY services.

PACE CNY is paid for by Medicare and/or Medicaid or private pay. If you are not currently enrolled in Medicaid, a financial assessment to help determine eligibility is completed free of charge.
To determine eligibility, applicants will be asked to:
Complete the applications and forms to help determine eligibility and cost for the participant.

Provide the following documents:

  • ID for applicant and Power of Attorney (POA)
  • Social Security card
  • Proof of citizenship (birth certificate, alien registration, or passport)
  • Proof of gross income (social security benefits letter, disability benefits and deductions, pension benefits and deductions, paycheck stubs, or other proof)
  • Proof of living expense (rent or mortgage payment, homeowner’s insurance and property taxes, utility bills, or other proof)
  • Proof of health care insurance (Medicare, Medicaid, or insurance cards)
  • Proof of assets (current bank statements, life insurance policies, burial contracts, stocks, bonds, IRA, annuities, or other proof)

Step 4: Enrollment

Receive notification of approval or denial of enrollment into the PACE CNY program. If enrollment is approved, schedule a meeting between the PACE CNY Team, the participant, and the participant’s caregiver(s) to discuss the partnership agreement and finalize plans for beginning PACE.

Upon enrollment, our Interdisciplinary Care Team completes an in-depth healthcare evaluation and develops a comprehensive, individualized care plan based on medical, physical, emotional, and social needs. The individual’s enrollment becomes effective on the first of the month
